Public Safety forms are available below. Please select, print, and complete the appropriate form. Forms may be submitted via email, fax, or at the Public Safety Department during office hours. Printed copies can be requested and picked up the same business day. All applicable payments are due at the time forms are submitted. Forms of payment accepted are cash, check and credit card (3% processing fee will be added for credit card transactions)
Dog License | The City of Minnetrista has decided to forego issuing dog licenses beginning January 2019 - so your dog does not need to be registered with the City
Permit to Purchase | Permit to Acquire applications can be submitted by email, fax, mail or by dropping them off at the Police Department during business hours. Once the background has been completed, you will be notified via mail if your permit has been approved. If approved, the permit will be mailed to you.
